Finger rigging orientation?

I've been baffled by the Joint placement/orientation, etc, as well. The whole process doesn't seem to work like other apps as far as clicking a button and everything is aligned properly. I always wound up experimenting a lot to get it to work.
 
The Joint Tool pretty much assures your Bone alignment will be anything but correct.
Use the Joint in the Scene Object's menu.

If the first two joints you place are aligned vertically or
horizontally, the Bone they create will be aligned as well.

So start with an aligned Bone and rotate it on it's Pivot/Joint to position for the
next Joint/Bone and so on and every bone will be aligned in Object Orientation.
 
Here's a classic example of an IK Failure.

To correct this just add a Pole, which will tell the Solver which direction to point the bones.
