How are people making height maps for displacement?

The title says it all. I'm wondering what applications and techniques people are using to make height maps for use with UV mapping and the Displacement modifier. I want to add some details to this sword hilt, and displacement is the only technique I can think of.

You need an image edit app which can handle 16bit color depth (not much on the market I guess: PS, Photoline, ???).
When you stay inside Cheetah3d you can try "Normal-map" or "Bump -map" instead. Displacements introduce a load of new geometry into the object BTW.
